Subject Access Request
Summary
A subject access request is simply a request under the Data Protection Act 2018 to an organisation asking for a copy of personal data that it holds about you.
Is there a fee for submitting a Subject Access Request?
No. However, the Trust can charge a ‘reasonable fee’ when the request is deemed ‘manifestly unfounded or excessive’ or if it is repetitive. Fees will be based on administrative costs of providing the information.

How to make a request
In order to make a request for copies of yours or someone else's information, please click on the relevant form from the new request section on this site.
To enable us to provide you with the information you are requesting, we are obliged by law to request two forms of identification. This is to protect your confidentiality.
Please provide one document from each of the following categories: -
driving licence or passport or birth certificate
and
bank statement or utility bill or credit card statement.
If you are requesting someone else’s information, you must also provide one document from each category confirming their identity.
Please ensure you have all the relevant documentation ready to upload at Section 3 and/or Section 4 of the subject access request form.
To request information in relation to yourself please use the form called - Subject Access Request (My own information).
To request information on behalf of someone else please use the form called - Subject Access Request (On behalf of someone else).
Prior to requesting information on behalf of someone else please ensure the person whose information you are requesting completes and signs the consent form which can be downloaded from the following link:

and upload the completed and signed form to section 7 of the subject access request form.
